Aceso Interactive, Inc., a leader in patient experience and engagement solutions, announced a collaboration with Alvee, an AI-driven, whole-person care navigation platform. Alvee’s AI services are now built into the Aceso Sphere™ platform, so hospitalized patients can share what matters to them, from social needs to wishes for care, then opt in to supportive services and receive follow up, all from the bedside.
Non-medical factors such as food insecurity, housing instability and lack of transportation are estimated to drive up to 80 percent of health outcomes. Yet social needs screening often depends on paper forms or manual intake steps that compete with clinical priorities. Needs get missed, and patients go home without the support that could keep them well. For older adults, hospitals now report to CMS on whether they ask what matters most, yet those conversations often live on manual whiteboards instead of the patient chart.
Aceso and Alvee address that gap by bringing whole-person care navigation to the screen patients already use: the bedside TV or tablet. Within Aceso Sphere, patients have a plain-language conversation with Lucy, Alvee’s What Matters agent, at a time that suits them. Lucy listens for social needs, mood and memory concerns, and wishes for care, then summarizes the conversation for the care team to review and confirm with the patient. Alvee documents those needs in the health record and builds a plan that spans health plan benefits, public programs, and community resources. Patients can then opt in to the services they want, and care teams can track next steps, including follow-up after discharge. Medicare reimbursement for that follow up helps hospitals sustain the work.
The integrated solution delivers:
“A patient’s health doesn’t stop at the hospital door, and neither should their care,” said Geoff Fiedler, Co-Founder and President of Aceso Interactive. “By bringing Alvee’s AI-enabled services into Aceso Sphere, we’re giving providers a simple, compliant way to understand the social needs of every patient and connect them with the services they need before they leave the hospital.”
“Identifying social needs and knowing what matters to a patient are only valuable if they lead to action,” said Nicole Cook, Founder and CEO of Alvee. “Aceso’s bedside platform puts our screening and services in front of patients at a moment when they have time to engage and care teams are ready to help. Together, we’re making sure what matters to each patient isn’t lost, that they get the support they need, and that they stay connected to their care team after they go home.”
